Providing Individuals With Constant Interaction

Goshen Healthcare Services Inc. are aware of the new change with the final settings rule with HCBS from CMS home-based community services. Goshen Healthcare Services Inc. are solely passionate in providing individuals we serve with both choice and constant interaction in the community. We want to make sure
that our individuals will grow as Goshen Healthcare Services Inc. will support them in the following areas:

  • Authority- to have say on what they want to provide in their person-centered plans.

Community Living

Person-Centered Planning, ensures all individuals are served the way they choose.

COMMUNITY LIVING – GROUP HOME SUPPORT

Goshen Healthcare Services Inc. proposes to support individuals with developmental disabilities and carry out the mission of Person-Centered Planning by making sure that all individuals are being served by our agency in the way that they choose. Choice is a preferred preference and Goshen Healthcare Services Inc. will do all it takes to assist them with their wants. Goshen Healthcare Services Inc. wants to exercise their PCP that are individually designed to respond to the person’s needs and desires, as well as to reflect an appreciation of the individual’s unique personality. Personal Support

Allowing participants to work toward independence and self-determination.

PERSONAL SUPPORT SERVICES

Goshen Healthcare Services Inc. will provide integrated support services to participants to allow participants to work toward independence, self-determination, inclusion, productivity, and integration, in all aspects of community life across their lifetime. Goshen Healthcare Services Inc. will support individuals as they focus on life experiences focusing on good quality of life. Goshen Healthcare Services Inc. will provide services that will build on each participant’s current support structures to work toward individually defined life outcomes, which focus on developing the participant’s abilities for self-determination, community living, socialization, and economic self- sufficiency. Our agency will provide services that supports integrated life domains that are important to a good quality of life, including daily life, safety and security, community living, healthy lifestyle, social and spirituality, and citizenship and advocacy.

Respite

Respite relieves families or primary caretakers from their caregiver duties.

PERSONAL SUPPORT SERVICES

Goshen Healthcare Services Inc. will provide quality respite Care for short-term care intended to provide both the participant and his or her family with a break from their daily routines. Respite relieves families or primary caretakers from their caregiver responsibilities while providing participants with new opportunities, experiences, and self-determination.

Goshen Healthcare Services Inc., will provide high quality respite care for you or your loved one, when you need it. Respite care provides temporary relief for family caregivers who are caring for people with disabilities, special needs or who are simply aging. Goshen Healthcare Services Inc. will also assist in recovery after a surgery or hospital stay. Caring for a loved one in the home can be a very rewarding experience but it can also be an overwhelming and exhausting undertaking. It is essential that family members get regular support in order to relieve stress, restore energy, and promote balance. Respite care from Goshen Healthcare Services Inc. provides short-term breaks that relieve stress, restore energy, and promote balance within a household. Our services can help to sustain a family caregiver’s health, delaying out-of-home placements.

Nursing Services

Delegation and case management services to our participants.

PERSONAL SUPPORT SERVICES

Goshen Healthcare Services Inc. Goshen Healthcare Services Inc. will contract Dimensional Health Care Associates Inc. (DDA approved nursing provider) to provide nursing services which includes delegation and case management services to our participants such as Nurse Consultation, Nurse Health Case Management, Nurse Case Management and Delegation that focus on health safety and practices to limit or prevent adverse outcomes. Nursing services are identified in participants’ plans which include goals for participants’ quality health outcomes. Nursing Support services include Nurse Consultation, Nurse Health Case Management, Nurse Case Management and Delegation under COMAR 10.27.11 (Delegation Nursing).

Community Development Services

Helping participants to develop and maintain skills related to community membership.

COMMUNITY DEVELOPMENT SERVICES

GHSI will provide Community Development Services (CDS) which are distinct habilitative services, separate from residential services, which aide the participant to develop and maintain skills related to community membership through engagement in community-based activities with people without disabilities. GHSI will provide quality Community Development Service to participant based on their person-centered plan. CDS will be provided different settings in the community including volunteering to help individuals learn new skills and keep the skills they currently have related to their individual goals and outcomes. Community Development Services will be provided Monday through Friday only and may not exceed a maximum of eight (8) hours per day.
